Care Community 

Northwich PCN supports and is supported by the wider Care Community . We work closely with other partnerships to ensure best patient care.

Our local Integrated Care Partnership describe a Care Community as 'People living, working or involved with a defined area of Cheshire West, who build on the strengths within their community to contribute to the health and well-being of local residents'.
